specialist will contact you shortly. This form is for quoting and scheduling
purposes only. Before we can begin calibration on the piece of equipment,
a Contract Summary For Customer Approval form MUST be filled out completely
and faxed (or sent with equipment) to us.
Per ISO/IEC 17025:1999(E) Section
4.6.3: Purchasing documents for items affecting the quality of laboratory
output shall contain data describing the dervices and supplies ordered. These
purchasing documents shall be reviewed and approved for technical content prior
to release.
NOTE: The description may include
type, class, grade, precise identification, specifications, drawings, inspection
instructions, other technical data including approval of test results, the quality
required and the quality system standard under which they were made.
For RF/Isotropic
Field Probe calibrations: Please review ISO Probe Calibration Services and fill out
the ISO Probe
Request Form and fax/mail back to us.
NOTE: For Probe Frequency Response and Channel Match
calibrations which go below 1 GHz, we deviate from IEEE 1309 in that a Tri-plate
is used for a transfer standard. Please refer to the ISO Probe Calibration
Services document for technical justification regarding this deviation.
